A patient care co-ordinator (PCC) is required to deal with patients and, if appropriate, their carer, before or after the patients consultation with a clinician or other healthcare professional.

Working closely with the patient and their clinician and/or other healthcare professionals, the PCC co-ordinates patients healthcare and supports them to access the appropriate service to ensure that they get the most suitable care from whatever health or social care provider is appropriate.

They will be given a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. This role will specifically work within the Acute Response Team providing a rapid response to patient who are acutely unwell on behalf of all 6 member practices but may shift on to other projects as services develop.

Main duties of the job

We are looking for someone who has experience working within a multidisciplinary team, supporting patient care through coordination of services. Role includes:

  1. To work within the Acute Response Team acting as first point of contact
  2. To collate all of a patients identified care and support needs and review the options to meet these needs and bring them into a single personalised care and support plan (PCSP)
  3. To help people to manage their needs by answering their queries and supporting them
  4. To support the clinical team to engage with wider health professionals to coordinate care
  5. Managing referrals
  6. To assist patients to be better prepared to have conversations on shared decision making

Job responsibilities

The following are the core responsibilities of the PCC.

There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

  1. To work closely with practice and PCN healthcare roles, the PCC is to work within the Acute Response Team acting as first point of contact
  2. To collate all of a patients identified care and support needs and review the options to meet these needs and bring them into a single personalised care and support plan (PCSP) in line with best practice
  3. To help people to manage their needs by answering their queries and supporting them in making appointments or accessing appropriate services
  4. To support the clinical team to engage with wider health professionals to coordinate care
  5. Managing referrals and ensuring the patient understands and inputs into the care plan
  6. To assist patients to be better prepared to have conversations on shared decision making and to improve awareness of shared decision making and related support tools
  7. To provide patients with high quality, easy to understand information to assist them in making choices about their care
  8. To liaise with other PCCs in other practices within the PCN and share best practice
  9. Where appropriate, to assist patients to access personal health budgets
  10. To provide coordination and navigation of patients, and where appropriate their carers, across health and social care services
  11. To support in the delivery of enhanced services and other service requirements on behalf of the PCN
  12. To lead in the management of patient complaints and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events
  13. To organise, attend and participate in the delivery of multi-disciplinary teams (MDT) within PCNs.
  14. To undertake all mandatory training and induction programmes
  15. To contribute to and embrace the spectrum of clinical governance
  16. To develop yourself and the role through participation in training and service redesign activities
  17. To attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
  18. To contribute to public health campaigns (e.g. flu clinics) through advice or direct care
  19. To maintain a clean, tidy, effective working area at all times

In addition to the primary responsibilities, the PCC may be requested to:

  1. Support delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives
  2. Undertake any tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner
  3. Duties may vary from time to time without changing the general character of the post or the level of responsibility

Qualifications

Essential

  • GCSE grade A to C in English and Maths

Desirable

  • Health and Social care qualification

Experience

Essential

  • Experience of working within a team of health and/or social care professionals

Desirable

  • Experience of working in primary care
  • Experience of an acute care setting/team/service
