Vacancies: Social Prescriber and Trainee Social Prescriber with Citizens Advice Wirral


Social Prescriber: 35 hours per week, salary £20,200
Trainee Social Prescriber: 21 hours per week, salary £11,744

An exciting opportunity has arisen for a full time Social Prescriber and a part time Trainee Social Prescriber to join Citizens Advice Wirral to help deliver the Wirral Social Prescribing Scheme. The Scheme is partly funded by the Department of Health and Social Care and will be delivered by team members from both Citizens Advice Wirral and Age UK Wirral.

We are looking for creative, innovative and community-focused individuals to join this partnership team. The Social Prescriber will work with GP patients to create personal action plans that increase the individual’s confidence, resilience and integration in the local community and improve their health and wellbeing.

If you have experience of working in the health, community or voluntary sector, experience of working with patients with mild to moderate mental health needs and take a person centred approach to empowering people to achieve their goals, this is the role for you!
