Age UK Wirral are Recruiting for a Lead for Out of Hospital Services

In the role of Lead for Out of Hospital Services, you will:

• Develop and lead on services that directly impact on health and social care patients – including (but not exclusively) Going Home Team, PCN projects and Social Prescribing projects.

• Oversee the operational management of these services – ensuring full staff coverage and dealing with any escalated issues that arise.

• Support staff and volunteers to ensure they are delivering the best service possible, feel proud of what they achieve, are involved in the development of their service and have a clear understanding of their goals and objectives.

• Coordinate and submit reports for commissioners and reports needed for internal purposes

• Attend external meetings that impact on the development of health-based services and feedback to relevant people within the organisation.

• Maintain the organisation’s role and profile in external relationships relating to health and social care services.

• Work as part of the leadership team, developing a good understanding of the wider organisation and providing peer support where appropriate.

Essential Criteria

This role requires the following skills/experience/qualifications in order to qualify for interview:

• Experience of managing/leading people.

• Experience of managing projects.

Part-time hours: 37.5 hours a week
